My signature below constitutes acknowledgement that: I, __________________________ consent to authorise Northern Rivers Electrolysis and any members of their staff to perform Electro Epilation, including Electrolysis, Thermolysis & Combined Currents for my hair removal concerns.

| I understand that several appointments may be necessary to complete the treatment | |
| Side effects and complications are minimal; however, I understand that I may experience erythema, bleeding, temporary scarring, swelling, headaches, appearance of small pimples or pustules, crusts, bruising, induration, Hyper or hypopigmentation or pitting. There may be a sensation of heat or tingling. | |
| It has been explained to me and I understand that my skin condition/s and hair removal will respond much better when part of an overall skincare program and I understand that compliance with my aftercare instructions will greatly affect my final result. | |
